Sore Throat: When It's Viral vs. Bacterial — and When Antibiotics Are Needed

Sore throat (pharyngitis) accounts for more than 12 million doctor visits annually in the United States alone, making it one of the most common reasons for seeking medical care. The challenge for both patients and clinicians is determining whether the cause is viral—requiring only supportive care—or bacterial, which may need antibiotic treatment. The vast majority—approximately 70-85% of sore throats in adults and 85-95% in children under age 5—are viral in origin and will resolve without antibiotics.

Viral Sore Throat: The Most Common Scenario

Viral pharyngitis typically accompanies other upper respiratory symptoms. The sore throat often develops gradually alongside runny nose, cough, hoarseness, and sometimes conjunctivitis (pink eye) or low-grade fever. Common viral culprits include rhinovirus (common cold), adenovirus, influenza virus, coronavirus (including SARS-CoV-2), Epstein-Barr virus (mononucleosis), and enteroviruses. Viral sore throats typically improve within 5-7 days with symptomatic treatment only. Antibiotics provide no benefit and may cause harm through side effects and promotion of antibiotic resistance.

Strep Throat: When Bacteria Are to Blame

Group A Streptococcus (GAS) is responsible for 5-15% of sore throats in adults and 15-30% in children aged 5-15. Unlike viral pharyngitis, strep throat requires antibiotic treatment to prevent potentially serious complications, including acute rheumatic fever (which can damage heart valves) and post-streptococcal glomerulonephritis (kidney inflammation). Key features suggesting strep include: sudden onset of severe sore throat, fever above 101°F (38.3°C), swollen and tender anterior cervical lymph nodes, tonsillar exudates (white patches), and notably, the absence of cough. The Centor criteria use these features to estimate the probability of strep infection and guide testing decisions.

Diagnosis and Antibiotic Stewardship

A rapid antigen detection test (RADT) performed in the doctor's office provides results in minutes and has high specificity (approximately 95%), meaning a positive result is reliable. A negative rapid test in children should be confirmed with a throat culture, which takes 24-48 hours but is more sensitive. This two-step approach ensures antibiotics are prescribed only when truly needed. When strep is confirmed, a 10-day course of penicillin or amoxicillin is the standard treatment. It is critical to complete the full course even if symptoms improve after a few days, as premature discontinuation increases the risk of rheumatic fever.
